I think, at least in norway it is normal to be on first names. never heard someone call teachers by the last name the 14 years i was in school

In Hungary we call them by their second name + say teacher after it, so like "Smith teacher man/ woman" ~ "Smith tanár úr" or "Smith tanárnő", depending on their gender. Furthermore, we have this thing called "magázás", which is kinda like "formal addressing". Germans have something like this too.
