you could do both, you can choose to include the combo items in an outfit or just have them be single items cropped ! As long as you inform us what the combo items are !

It's basically when you take two (or more) items of clothes (hair/shirt/shoes/etc) and create a new item. 
You're just layering clothes over top of clothes to make a new item basically
