the only way i can explain this is that u can have both, but since ur rate of lipolysis is so low ur body cant utilize the high insulin sensitivity so the effect kinda gets nullified, but when the patient uses a medication that basically stimulates lypolysis, their body starts rapidly losing weight since u dont have the means to keep it because of the insulin sensitivity, so involuntary weight gain becomes involuntary weight loss
