U can make new items for a new base, but changing all the existing to fit would be a work, way to big for the unpaid designers.

No because most items are designed to fit the specific base they've been designed on, either male or female, and they're super different from each other, it's physically impossible to add a gender neutral avatar option without either redesigning everything or designing new stuff for that specific base
