My friend is struggling with an eating disorder
She is obsessed with being thin and works out constantly, pukes food she consumed that is sugary, and she seems proud of it as well
for example she told me she doesn't get her period anymore and it seemed like she thought that was a really cool thing of her body 

She just sent me a text that said "when you saw me last Thursday, did you think I looked different? did I look thinner or something? or did I look more? anything?"and how in the hell am I supposed to answer that?I don't know what she wants to hear, I don't want to say the exact opposite she might like and I definitely don't want to be triggering because I think she is very fragile 
mentally 

I have absolutely no experience about anything like this and I don't want to do something that might be harmful to her... how do I approach this, what do I answer?

I would maybe talk to her about your concerns but usually anorexic people are not open to talking about weight loss being a bad thing and how it affects their health negatively.

It really depends on whether she's a minor or not?? And if she's a minor, if she has supportive parents or guardians??

I think she wants you to reply that she is thinner. But I would say that "you look slim" and that you need to seek help for an eating disorder is not good.
She won't want you to tell anyone, but ask her to talk to her parents and a curator/psychologist/nurse who can help her and then send them to an eating disorder unit. Then you can write that you are there as a support for her for these people need to have someone by her side to cope up to a normal weight.

You can try to ask her to come home to you and ask her to choose a food that she likes and then talk to her about other than food and the body so she can get something else to think about.
Tell her to go to the toilet before the food and then make sure she doesn't go in there after the meal and wait for 1h and then maybe you can play some game so that she sits still.

Eating even if thoughts and anxiety come is the best way to cure an Anorexia. When she eats, there should be someone sitting next to her.

reeks of pro-ana, legit let her know you'll not indulge her in this and that she should seek help.

i won't say it's attentionseeking, but she seems like she's desperate for someone to notice, both the weightloss and her sickness. there are some underlying issues here, i think she's dealing w something that she uses the ed to cope w. 

what helped me when i was similar like that, was to be told off, to be told the dangers of this shit disorder and well to get help. i'm still suffering from an ed, (more likely to do w depression as i forget to eat/don't bother to eat)  to the point that i get heart palpations, i haven't had my periods in months, my skin is brittle and dry, i'm always dizzy and standing up can take away my breath.  like this shit aint fucking pretty.

yes. but also tell her that you are her friend and she can talk to you if she has any problems. but i guess whatever you say concerning how her body looks will be difficult and its best to not say anything about how she looks.
